Abstract:This thesis investigates medication wastage within the Clinical Pharmacy department at Isala Zwolle, aiming to identify causes and propose solutions to enhance sustainability. A mixed-method approach was employed, including a comprehensive literature review and empirical research through interviews with pharmacists, nurses, and other stakeholders. Key findings indicate that approximately 30% of medications prepared are returned unused. Major causes of wastage include over-preparation for weekends, regulatory constraints preventing reuse of returned medications, and inefficiencies in inventory management. The study also highlights differences in wastage rates across various departments and times, with higher wastage during weekends due to over-preparation. Recommendations focus on improving inventory tracking systems, enhancing staff training on sustainable practices, and integrating technology such as electronic prescribing systems. Additionally, fostering better communication and coordination among hospital departments and involving patients in responsible medication management are crucial strategies. The research emphasizes the importance of recent regulatory changes that allow for the safe reuse of medications, which have already led to significant reductions in wastage. By implementing these strategies, Isala Zwolle can enhance its sustainability and contribute to a more efficient healthcare system.
