ThyroMap: the tool to improve the transition process to adult care for children and young adults with a thyroid disorder, their parents, and pediatrician.

In this thesis, the transition process to adult care for children and young adults with thyroid disorder, is investigated. With this knowledge, supporting technology has been developed, to improve this transition process, and prepare children with thyroid disorder for adult care. This research has led to the development of an app, providing a roadmap that helps children, their parents and their pediatrician, in the transition to adult care.
