Abstract:Cartilage has a limited ability to regenerate without treatment. For large cartilage defects, two interventions are currently available: Autologous Chondrocyte Implantation (ACI) and conservative therapy. A new intervention is Instant MSC Product accompanying Autologous Chondron Transplantation (IMPACT), a treatment which is expected to result in lower costs and lower patient burden. The purpose of this study is to explore the cost-effectiveness of IMPACT compared to ACI and conservative therapy with in-man trial data. Data from 86 patients were included in the analysis. The results consisted of total costs and quality-adjusted life years for each strategy, which were compared via a fully incremental analysis. A breakdown of costs was provided to determine the proportion of health consumption and productivity loss costs from total costs. The results showed an increase in both total costs and health effects for IMPACT and ACI, compared to conservative therapy. The breakdown of costs showed that for all three interventions, more than 70% of the total costs resulted from health consumption costs. For IMPACT and ACI, the largest part of these health consumption costs were attributable to surgery costs. This study found promising results that IMPACT and ACI could be cost-effective compared to conservative therapy, when data over a longer follow-up period would be included in the analysis.
