Abstract:Rheumatism is a chronic disease and it causes impairments in the daily life at different domains, for example in the social environment and at work but it has also influence on the psyche. The patients have to learn to live with several impairments because the disease can cause problems with the pursuit of particular goals. One way to promote the adaption to the disease is goal management. In this study goal management consists of these four strategies: goal maintenance, goal adjustment, goal disengagement and goal reengagement. The occasion of this study was, compared to preexisting surveys, the improvement of the measurement of goal management by the use of vignettes. Vignettes are little stories about several problems that rheumatism patients can have. There were several research questions: is it possible to recognize the 4 strategies in the responses of the sample, is there a kind of new strategy? Which strategy is mentioned most often and is there a preference for one or more strategies? It was also considered if al impairments of rheumatism patients are included in the vignettes and if there is a connection between a preference for a particular strategy and the rate of adaption of the disease. The sample consisted of 223 patients of the policlinic “Medisch Spectrum Twente”. The respondents had to fill in 4 vignettes and they had to invent own solutions. The Hospital Anxiety and Depression Scale was used for the measurement of the degree of adaption. The results showed that all of the 4 strategies in the participants` responses could be recognized. There was no new strategy detected. The strategy “goal adjustment” was the most mentioned (50%) strategy. Also this strategy was, together with “goal reengagement”, preferred. The respondents mentioned in addition new impairments that could be caused by rheumatism, for example: not being understood by other people and loss of control over life. There existed a positive connection (rs = 0,247, p< 0,05) between the preference of “goal disengagement” and the attendance of depressive symptoms. Finally it can be said that the use of vignettes are a good method for the measurement of goal management. .A preference for the strategy goal adjustment and goal reengagement exists. The shifting of the vignettes to the beginning or middle of the survey and the adding of further vignettes which contain other patient’s problems can form an improvement for future research.
