Abstract:Colorectal cancer is the third most prevalent and deadly cancer worldwide. In 10% of cases a positive resection margin is found after surgery leading to higher recurrence and lower survival. The current gold standard, histopathology, is unable to provide intra-operative assessment of resection margins leaving the surgeon unable to prevent positive resection margins during surgery. Methods are developed for using diffuse reflectance spectroscopy (DRS) and ultrasound (US) for intra-operative assessment of the resection margins. DRS and US measurements were acquired from ex vivo colorectal specimens of 97 patients directly after resection. Deep learning was used with DRS and US for tissue type classification of healthy colorectal wall, fat, tumor and fibrosis. In addition, US was used for semantic segmentation with transfer learning and ensemble learning. DRS was able to classify tissue types with a MCC of 0.40 while US was able to classify the tissue types with a MCC of 0.23. Ultrasound segmentations yielded a DICE similarity score of 0.84 and error in the top margin estimation of 0.77 mm. Results show promising use of DRS and US for intra-operative assessment of the resection margins in colorectal surgery for intra-operative guidance to improve patient prognosis.
