Abstract:The rising incidence of cancer in the Netherlands necessitates efficient and high-quality care. To address this, the Dutch government plans to centralize high-complexity oncological care. This study focuses on the allocation of cancer surgeries within the OncoZON network, with Catharina Hospital Eindhoven as the problem owner. The challenge lies in uncertain volume norms for different tumor types and their potential increase, making it difficult to determine the best allocation strategy. A mixed-methods approach incorporating literature research, interviews, and mathematical modeling was adopted. The results demonstrate the model's ability to find feasible surgery allocation solutions for various norm combinations. However, centralization leads to increased ICU, OR, and Ward capacity requirements across multiple hospitals. Patient perspectives indicate reduced travel time but limited freedom of choice. Hospital systems face challenges in capacity shortages, staff, and material requirements. The study highlights the trade-offs and implications of centralization for patient, hospital, and medical profession perspectives. The findings contribute valuable insights into surgical allocation under uncertainty and emphasize the need for regional and national decision-making focused on quality evaluation.
