Abstract:Objectives: In patients with resectable non-small cell lung carcinoma (NSCLC) with a diameter less than 2 cm, uVATS segmentectomy is the preferred surgical approach. Preoperative planning for this procedure is essential due to the complex segmental anatomy and the difficulty localizing the tumour intraoperatively. In Zuyderland Medical Centre (ZMC), preoperative planning is achieved using its in-house protocol for 3D lung reconstructions based on CT scans. The purpose of this study was to improve the in-house protocol, to allow labelling of the segmental bronchi, estimation of the bronchopulmonary segments and virtually simulate a segmentectomy. Conclusion and discussion: The renewed protocol for 3D lung reconstruction allowed accurate semi-automatic labelling of the segmental bronchi, which was less time-consuming than manual labelling, as well as virtual simulation of a segmentectomy. Since the estimation of the bronchopulmonary segments was inaccurate and not verified by an expert, researchers should further improve this method and validate it by professionals. Because the connective tissue between the segments is not distinguishable in a CT scan, structures that are visible, which are the arteries, veins and bronchi, could be used in this method.
