Abstract:This thesis investigates the impact of the Connected Care intervention, particularly the Virtual Care Centre (VCC), on the Outpatient Parenteral Antimicrobial Therapy (OPAT) care pathway in department X at Isala Hospital. In response to demographic changes and staff shortages, Isala implemented the Connected Care Centre to extend hospital-level care to patients' homes, with the medication@home project serving as a case study. Using a redefined capacity assessment framework, the study explores key performance indicators such as Length of Stay, Duration of OPAT, Total working time of healthcare professionals per patient, Wage costs per patient, and Number of Readmissions. Results show apparent improvements in the OPAT care pathway, including streamlined antibiotic delivery and reduced Length of Stay by 1.4 days. Despite limited statistical significance due to small sample sizes, the study recommends continuing the use of the VCC. Challenges such as a communication bottleneck underscore the need for ongoing improvement. This research contributes to the quest for a more efficient healthcare system and emphasizes the potential for cost savings with VCC implementation.
